I had a similar experience but in my case the club name appeared OK (Alto  
Velo Racing Club) but the Team name was missing (Webcor/Alto Velo). The club has 
 since renewed its license for 2007.  I have a message pending with USA  
Cycling about how to "repair" my license but haven't heard back from them  yet.
